Thank you for your interest in NIS for the education of your child. Below are forms that are available online for your convenience. However, since NIS accepts students at all times of the year, availability in certain classes may be limited and you are encouraged to contact the Development Office prior to submitting any admissions documents to make enquiries concerning availability. Bus maps and routes are NOT available online; please enquire directly with the Development Office.

If you prefer, you may also use the online request for an Admissions Information Packet, which has many of the documents listed here along with other printed materials, and we would be happy to send it to you via the postal mail. Progress Reports, Grades/Transcripts:

• In addition to the forms listed below, all applicants must submit grades or reports from the past three years with the application materials, and must also provide a final report upon arrival in Japan. High School applicants must submit official records in a school-sealed envelope or arrange to have them sent directly to NIS via postal mail.
