• Application for Admission Form

Please submit one application form per child, completed in full (front & back)

 

• Transcripts, Records & Reports

Requirements for records and reports differ depending on the grade of the applicant. (All documents must be in English; please arrange to have records translated if needed.)
  • ELC Preschool/Kindergarten:  If the applicant has attended a preschool in the past, please obtain reports from that school and submit them together with the  application materials. 
  • Elementary & Middle School:  Applicants must submit school progress reports from previous schools from the past three years. Reports should be submitted with all other application materials or sent directly to NIS in a school-sealed envelope.
  • High School:  The applicant must arrange to have official transcripts sent directly to NIS from the applicant’s school. Certified transcripts can also be submitted with other admissions materials but must be in a school-sealed envelope.

 

• Letter of Recommendation

NIS has two forms for recommendations, one for preschool applicants and one for elementary through high school applicants.

  • Elementary students  should request a recommendation from their current classroom teacher or principal and submit it together with the application materials in a school-sealed envelope;
  • Middle school students  should request a confidential recommendation from their guidance counselor or principal to be sent directly to NIS.  

 

• Admissions Health Form

This form provides information on the student’s medical history and is required for each applicant. NIS highly recommends that all students have a full medical check-up prior to enrollment. A form to present to medical professionals is available upon request.

 

• Curriculum Differences Form

One form per family.

 

• ELC Questionnaire

This form is for ELC preschool and Kindergarten applicants only.

 

• Photographs (2)

  • Student Photo:  Please attach a 3 x 4 cm, passport-style photo to the application form in the space provided in the top right corner.
  • Family Photo:  Please Include a photo of the family expected to be living with applicant during the time of enrollment for identification purposes; a casual “snap-shot” photo is acceptable.

• Passport Copy

Photocopy of the applicant’s passport is for verification of date of birth and nationality.

 

• "Juminhyou" Copy

Photocopy of the Residential Registration is required for residents of Japan only.

 

• Application Fee (¥20,000 yen)

Payment can be made via wired bank-transfer, invoiced, or paid in cash. The payment must be in yen, and the school cannot accept credit
cards. Applicants can request this fee to be invoiced with initial tuition and fee billing. Please contact the Development Office for wire-transfer information.

 

• Additional Reports

Any other reports or evaluations which may provide further information regarding the academic performance of your child and/or any other needs may be submitted.

 

 

We appreciate your cooperation in preparing these documents and submitting a complete admissions application. These documents provide an academic and social profile of your child and assist the admissions committee in making an informed decision.
